What is Solutions for Wellness?
A:
Solutions for Wellness is a free 6-month program designed to help you make step-by-step changes in your everyday life so you can improve your overall health. By joining, you'll receive personalized information to help you make healthy changes in your diet, exercise, stress management, and sleep habits.
Q:
What does it mean that the program is "personalized"?
A:
This means that the program material you receive is prepared just for you. The answers you give on your enrollment form and surveys are used to create your lifestyle management plan, meal and exercise plans, and regular progress updates.
Q:
How can I enroll in the Solutions for Wellness program?
A:
You can
click here to enroll online. Or you can enroll by mail or over the phone by calling us toll-free at
1.800.811.2592. When you enroll, you will be asked to answer questions that will help us develop materials just for you.
Q:
What if I can't find the enrollment number on the enrollment survey?
A:
Please look at the back page of your survey. Your enrollment number should be at the bottom left-hand side of the survey. Or, if your doctor gave you a Solutions for Wellness referral sheet, check for the enrollment number in the lower left corner, underneath the prescriber's signature.
Q:
My enrollment form number is invalid. What should I do?
A:
The number you provided may already be in use by another member. Please talk to your doctor about getting an enrollment survey. The survey will have an enrollment number printed on the back page. Each enrollment number can be used only once.

What will I receive after I enroll in Solutions for Wellness?
A:
When you enroll, you will receive a Solutions for Wellness Lifestyle Management Plan—based on the answers you give on the enrollment survey—and a Program Starter Kit to hold your program materials. You'll also receive your first follow-up survey. Every time you answer a survey about your health habits, we'll send you an update on your progress. These updates are designed to give you support and help you stay on track with your goals. We'll also send you newsletters full of practical information on how you can manage your diet, start and stick with an exercise program, handle everyday stress, and learn healthy sleep habits.
Q:
When can I expect to receive my program materials?
A:
You should receive your personalized information packet within 1 to 2 weeks of enrollment. It will include: Your Lifestyle Management Plan, a Program Starter Kit, and your first follow-up survey.
Q:
Does it cost anything to use the Meal Planner and Exercise Planner online?
A:
No. These tools are offered free of charge, just like all other features of Solutions for Wellness. However, you must be enrolled in the program to use these features.
Q:
Can I use the Meal Planner if I have special dietary needs?
A:
Yes. The Meal Planner can be used to create special menus for those who have diabetes, high blood pressure, and high cholesterol.

What if I misplaced my login and password for online access to the menu/exercise planners?
A:
If you've forgotten your password, follow the instructions at the
log-in. If you're still having problems, or forgotten your username, call us toll-free at
1.800.811.2592.
Q:
Can I stop participating in Solutions for Wellness?
A:
Yes, you can stop being in the program any time by either calling us or writing to us. However, we like to see our members succeed, so we provide support and encouragement throughout the program to help you reach your goals.
Q:
Is my information kept confidential?
A:
Yes. This program is being conducted by Lilly USA, LLC. by an independent program administrator, the Patient Marketing Group, LLC (PMG), and its vendors. Your personal information will only be used by PMG and its vendors to contact you to build your personalized wellness program. Your data may be combined anonymously with information from other participants to evaluate the results of the Solutions for Wellness program.
